Community Health at the Doorstep

Aapi Outreach Model.

The Aapi Outreach Model trains trusted local women to deliver maternal health, family planning, and nutrition support in underserved urban areas. Equipped with smartphones, Aapis geo-tag households, track health needs, and link families to clinics. They earn stipends for their outreach and often grow small side businesses, making the model both a health service and a path to women’s empowerment.

With only USD 4 per household, Aapis reached 124,000 families, raised CPR from 32% to 46%, and reduced fertility by nearly one child per woman.

Aapis improve community health while earning from tailoring, sales, or freelancing, often making USD 100–850/month alongside their outreach stipends.

At 10x lower costs than rural LHWs, Aapis link communities to clinics, increasing private patient volumes by 20–40% and sustaining urban health systems.

Our theory of change is that when children and adults gain access to knowledge, technology, and supportive learning environments, they develop the critical skills to break cycles of poverty. At AHKF, we design and test programs that make this transformation possible in low-income communities.

Primary Healthcare (PHC) Referrals

Connecting underserved families with government and private clinics for essential care.

We bridge communities and clinics, ensuring timely referrals, preventive care, and sustainable health access.

Nutrition & Malnutrition Screening Program

Tackling stunting and wasting through screening, counseling, and fortified food solutions.

We identified and treated 1,150 severe malnutrition cases and counseled over 6,000 families.

Vaccination Campaigns

Partnering with government vaccinators to protect thousands of children against preventable diseases.

We helped vaccinate 33,318 children, raising coverage in high-risk urban poor neighborhoods.

Digital Health Tracking & Data Systems

Geo-tagging households and logging real-time health data with mobile tools.

We created a live digital health map, tracking women’s health needs with precision.

Rawan Desk (Mental Health Counselling)

A trusted platform for counseling, feedback, and linking families to health services.
